• Explore the scope of services encompassed by the 2015 CPT code(s).
• Determine which members of your clinical support team are eligible to provide care management.
• Learn how to establish an effective practice workflow to capture monthly services.
• Understand documentation needed to support reporting of chronic care management codes.
